 Scheduling An Appointment

 All appointments are managed by the client and should be arranged through the appointment portal. Due to the importance of every minute, please try to arrive on time or communicate delays to Kristi, if possible. Even if appointments don’t start on time, I try to end on time. If, for some reason, I am delayed, clients will receive their full session allotment. For no shows, there will be a fee of half the cost of the appointment. It is preferred to give a 24 hour notice for any cancellations, so that someone else may be afforded the opportunity to have that spot, however, it is understood that sometimes things happen and it’s not possible to give that much notice (but, when possible, please let me know as soon as possible, if you must cancel). 

Money Matters

Created To Bloom does not process insurance. All fees are to be collected prior to the appointment (digital payment) or at the conclusion of the appointment, in person. 

If a client chooses to pre-pay for multiple sessions or a package deal, this fee can not be reimbursed, even if the full amount of sessions is not redeemed. Likewise, if a group of sessions is prepaid as a gift for another person, that payment can not be reimbursed to the benefactor or recipient, if not used. Prepaid packages expire in one year from date of purchase. If a gift package is purchased by a third party, there are no rights or provisions granted to the benefactor, including confidential client information or confirmation of appointments scheduled. 

Fee schedules may periodically change and current clients will be “grandfathered” in with their original fee amount for a period of six months and then their rate will increase to the new fee schedule. 

There will be an additional fee of $45 for any returned checks. For credit card processing onsite, there is an additional $2.00 fee to cover the processing charge. 


Confidentiality

All sessions are fully protected by confidentiality. Any notes taken are sealed, unless permission is granted to share with an outside entity. I do have a legal duty to report any abuse to a child (please see https://www.scstatehouse.gov/sess122_2017-2018/bills/4705.htm for more details), and suicidal or homicidal ideation. My clients will not be caught off guard and if there is any reason for concern, it will be discussed before any action is taken. If there is a need to release information, the client will sign a “Release of Information” form for each request.
